When the Department of Public Health publishes an official report about a person’s health, it is called a “Real State Agency” report. The official names for real state agency reports are: Health Information Summary Report, Health Information Report (HIS), and Personal Health Record (PHR).
A person can request a state agency report from the state health department as long as it contains information that was made available to the public by the public. If the report does not contain any personal health information, it cannot be labeled a REAL State Agency Report.
An example of a REAL State Agency Report is the Health Information Summary Report. The Health Information Summary Report contains general information about a person’s health status. If the state health department decided to include personal health information, they would have to make an additional request to the office of the secretary of State of Texas and they would also have to pay a fee to access the personal health records.
The Health Information Summary Report includes a number of factors considered when compiling the report. Some of the factors that are considered in compiling a REAL State Agency Report include the age of the person (if at all possible), their race, sex, date of birth, current address, family history of cancer and all family members with a history of cancer. Other factors that are considered in compiling a REAL State Agency Report include: if the person has ever had cancer, how many times since they first had cancer, how many types of cancer did they have, how many people were affected by the cancer, how long after they first had cancer did they have treatment, if the person ever had a serious illness, how long did the person have a serious illness, how long did the person last without treatment, how long did the person have treatment for the serious illness, if the person has ever had any surgery, if the person has ever been a smoker, if the person has ever been a heavy drinker, if the person is married or single, if the person has ever had or is currently being treated for mental illness, if the person has ever worked as a child care provider, if the person ever had a driver’s license, if the person ever had a job, if the person works at a factory, if the person has ever had or is currently undergoing a hearing impairment, if the person ever had a toothache or any type of gum disease in the mouth, if the person has ever had or is currently undergoing any other condition where there is a risk of infection or a need for hospitalization or other treatment, and any special needs or medical issues .Visit:- https://danhkhoireal.vn/the-aston/
